【摘要】 目的 为心肌缺血／再灌注损伤所致心性猝死的法医学诊断提供形态学依据，方法运用免疫组织化学方
法，研究bax蛋白在心肌组织中不同区域的表达。 结果 对照组正常心肌组织无bax蛋白表达。bax蛋白再灌注0．5
h后即可在缺血区域表达，且表达主要在心肌内层。再灌注1 h后可观察到bax蛋白不仅表达于心肌内层且已经扩展至
心肌的全层。再灌注2 h发现心肌外层表达明显强于心肌内层，随着再灌注时间的延长可以发现bax蛋白的表达逐渐减
弱。 结论在急性心肌缺血再灌注中，bax基因对心肌细胞凋亡的发生有着重要的作用，且可以作为心肌缺血再灌注
的早期诊断提供一项辅助诊断依据。

【abstract】0bjective to search the ive morphologic evidence for the forensic pathological diagnosis on the acute
myocardial ischemia／reperfusion in rats． method to study the of bax protein in the different rid& in the acute
myocardial ischemia／reperfusion(i／r)in rats with immunohistochemistry． result these was no of bax protein
in control group．the of bax protein began to increase in myoc ytes 0．5 h after i／r in the ischemic areas and the ex—
pression of bax protein in inner was stronger than in outer ．however，the of bax protein appeared firstly
in the inner of myocardium and extended gradually toward the outer 1 h after i／r．there would find that the expres —
sion of bax protein in outer was stronger than in inner ，then the of bax protein was weaken gradually．
conclusion m aybe bax gene is a important regulative gene in the proc ess of apo ptosis，these changes observed in the present
sudy might be useful for the f0rensic pathological diagnosis of the early myocardial ischemia／reperfusion．

缺血能导致组织损伤甚至细胞死亡，而缺血后再灌注又能
引起新的损害，即所谓的再灌注损伤 j。心肌缺血再灌注损伤
经历两个独特而又相关联的病理过程：内皮触发与中性粒细胞
放大。再灌注早期的主要病理生理改变是内皮细胞功能失调，
它以内皮一氧化氮释放明显降低为特征，这种内皮功能状态的
改变可引发一系列的病理生理变化，开始再灌注损伤的病理过
程 2 j，这就是所谓的“内皮触发机制”；紧接着再灌注过程中的第
二个主要病理生理改变即“中性粒细胞放大作用”。
本实验，通过观察大鼠心肌组织，在心肌缺血后再灌注的不
同时间段的bax蛋白的表达规律，以期能为心脏缺血再灌注损
伤及其引起猝死的法医学诊断提供依据。
